An Advance Plan sets out an agreed plan to be followed when the child’s condition deteriorates and it is considered the child is unlikely to survive for longer than 6- 12 months; it is specific to the individual child and their views and wishes are where it is considered appropriate are included, the plan is completed with full collaboration with parents or carers and medical teams.
It provides all individuals, school, respite, carers in the home with a full appreciation of what is requested to happen at the time of their child’s death; if, for some reason parents are not with their child at that time, then staff caring for them are fully aware of the families wishes.
